Op dit moment nemen wij geen nieuwe patiënten aan

Om goede en toegankelijke huisartsenzorg te kunnen blijven bieden aan onze huidige patiënten, hebben wij tijdelijk een inschrijfstop.

De inschrijfstop geldt tot 1 november 2026. Vanaf dat moment verwachten wij de inschrijving voor nieuwe patiënten weer te openen.

Bent u al patiënt bij Huisartspraktijk Aletta? Dan verandert er uiteraard niets voor u.

Inschrijven bij huisartspraktijk Aletta.

U en uw gezinsleden kunnen zich bij ons inschrijven als patiënt.
Hiervoor moet u wel in ons postcodegebiedt wonen.

Waarom kan ik mij niet buiten het postcodegebiedt inschrijven?
Om ook in een medische noodsituatie voor u klaar te kunnen staan moeten onze artsen binnen 10 minuten per fiets bij u thuis kunnen aankomen.
Helaas kunnen wij daarom geen personen buiten onze postcodegebiedt inschrijven.

Inschrijven in 2 stappen.

Stap 1
Controleer hieronder of uw postcode binnen ons postcodegebiedt valt.
Als dit zo is dan kunt u verder gaan naar stap 2.

Stap 2
U kunt zich op twee manieren bij onze praktijk inschrijven:

  • Optie 1 U kunt zich online inschrijven door hier te klikken;

  • Optie 2 (op de praktijk): Print het formulier uit en vul het in. U kunt het daarna bij ons inleveren. Heeft u geen printer? U kunt bij de assistente een geprint formulier ophalen.

 

Hulp nodig bij het inschrijven?

Vindt u het lastig om het formulier zelf in te vullen? Wij helpen u graag! Bel ons eerst even om een afspraak te maken. Dan houden wij rekening met uw komst.

  • Zodra u het formulier heeft ingeleverd, gebeurt het volgende:

    1. Afspraak maken De doktersassistente plant voor u een afspraak in. Dit is een kennismakingsgesprek met de huisarts of de verpleegkundig specialist.

    2. Uw medisch dossier De assistente vraagt uw medische gegevens (uw dossier) op bij uw vorige huisarts. Zo zijn wij direct goed op de hoogte van uw gezondheid.

    3. Het gesprek U komt naar de praktijk voor het gesprek. Wij horen dan graag hoe u over uw gezondheid denkt en wat u van onze praktijk verwacht.

    4. De inschrijving afronden Na het gesprek maken we de inschrijving definitief. De doktersassistente laat aan uw zorgverzekeraar weten dat u nu patiënt bent bij onze praktijk.

 
Temporary registration stop

We are currently not accepting new patients

To ensure that we can continue to provide good and accessible healthcare to our current patients, we have temporarily stopped accepting new patient registrations.

The registration stop will remain in place until 1 November 2026. We expect to reopen registration for new patients from that date.

Are you already registered as a patient at Huisartspraktijk Aletta? Nothing will change for you.

Register with Aletta General Practice

You and your family members can register with our practice as patients. To register, you must live within our practice area.

Why can I not register if I live outside the practice area?
To ensure that we can provide medical care when you need us, including in an emergency, our doctors must be able to reach your home by bicycle within 10 minutes.
For this reason, we are unfortunately unable to accept patients who live outside our practice area.

You can register with our practice in one of two ways:

  • Option 1 – Online: You can register online by clicking here.

  • Option 2 – At the practice: Print the registration form, complete it and hand it in at our practice. If you do not have access to a printer, you can collect a printed copy from our medical assistant.

Need help registering?

Do you find it difficult to complete the registration form yourself? We will be happy to help you.
Please call us first to make an appointment, so we can make sure someone is available to assist you when you visit.

  • Once you have submitted your registration form, the following will happen:

    1. Making an appointment
      Our medical assistant will schedule an appointment for you. This will be an introductory consultation with a GP or nurse practitioner.

    2. Your medical records
      Our medical assistant will request your medical records from your previous GP. This allows us to become familiar with your medical history and healthcare needs.

    3. The consultation
      You will visit our practice for the introductory consultation. We would like to hear about your health, any healthcare needs you may have, and what you expect from our practice.

    4. Completing your registration
      After the consultation, we will finalise your registration. Our medical assistant will inform your health insurer that you are now registered as a patient with our practice.
